Child Enrollment Documents
To begin your child’s journey at Bright Beginnings Learning Center, parents must fill out our Child Application, Health and Enrollment form, Routine Field Trip Permission, Picture Permission, Family Information, Emergency Contact form, CACFP Enrollment Form and CACFP Income Eligibility Form.
All of these documents must be submitted before a child’s first day. Families may pick up an enrollment packet at our center or print one directly from our website below.

Please note that completing the required paperwork does not guarantee enrollment. A confirmation from the administrator is necessary to finalize your child’s placement, ensuring we have the proper availability and resources to offer the best possible care.
Ages & Stages
Additional Tool
Ages & Stages is a widely recognized tool designed to assess and track children's development in key areas, including communication, problem-solving, social-emotional, and fine and gross motor skills. By providing a clear picture of each child’s progress, Ages & Stages ensures that any potential concerns are identified early so that children receive the support they need to thrive.
At Bright Beginnings Learning Center, we use Ages & Stages in all our classrooms—from infants through Pre-K. Our educators regularly monitor each child’s growth and milestones, tailoring activities and strategies to each child’s unique needs. By integrating this assessment into our daily routines, we create a responsive and nurturing environment where children can continuously flourish and parents stay actively involved in their child’s development.
